New Parishioner Registration Form

Welcome to St. Anselm Catholic Church!  We are happy you are here with us!  

To register as a parishoner, please use this online form.  

During registration, please select "St Anselm Catholic Church, Madisonville" as your organization. Please enter your information as completely and accurately as possible.  The more accurate our information is, the better we can serve your needs.

If you are unable to register through My Own Church or you would like some help, please contact the parish office and we will be happy to assist you.

Soon after registration, you will receive envelopes quarterly, your first set should arrive in about 2 weeks.
